  • Each contestant must read and understand all. Rules & Regulations and sign all entry forms to be a contestant in the Jim Territo Charity Tournament 2008. Boat Captains can sign for their Team.
  • It is the Boat Captains responsibility for their team.
  • This is a one-day tournament, open to any who wish to participate.

LIABILITY RELEASE

In signing this agreement, I hereby release the organizers (Huron Valley Sport Fishing Club) and Down River Walleye Federation – (hereafter referred to as D.W.F), its officers, and employees, its sponsors and their agents from any and all damages, claims, demands, costs or expenses relating to injuries, death, damage, theft, fire or any loss of any persons or any property which I may sustain and or which I may cause by reason of participating in or in connection with this tournament. I further release (Huron Valley Sport Fishing Club, DWF), et all for all damages relating to adverse weather conditions while participating in this tournament. It is my responsibility to seek safety when adverse and/or unforeseen weather conditions occur prior to, during or after the tournament. I signify by my signature that I have read, understand and agree to the above.

Keep this Section and Present it at the Launch and Weigh-In

 

Top portion with payment to be turned in at Huron Valley SportFishing Club or DWF meetings. You cam also write a check or M.O. made out to the “Huron Valley Sport Fishing Club”, then mail to Billie Jo Cruz, at 18616 Koester St., Riverview Mi, 48193. No entries after 8pm on April 9th, 2008.

 

Entry Fee: $20 Per Person

Times: 7:00 am to 2:00 pm

Weigh-in to be held at Elisabeth Park

Weigh-in begins at 3:00pm
